Facilities Request Form

I acknowledge:

I have read and agree to the terms outlined in the Facilities Use Policy and the Facilities Fee Schedule.
Non-FUPC sponsored events organizers further agree to the conditionally refundable $100 Security Deposit.
I will supply a valid Certificate of Insurance before the start of the event.
FUPC events shall have priority to all other events.
FUPC shall be held harmless for such schedule changes.
Outside doors may not be propped open at any time.
Assistance with set up is available.
Long term leases and rentals are approved under a separate contract.
